TITLE OF POSITION: Case Manager
NATURE OF POSITION: The Case Manager is an integral member of the interdisciplinary
health team and is responsible for assisting with tasks that are difficult and time consuming for clinic staff to address.
QUALIFICATIONS:
- High school graduate required.
- Healthcare experience required.
- Possess strong clinical and teaching skills.
- Able to assume a leadership role in a team environment.
- Strong interest in working with an interdisciplinary health team and culturally diverse patient population self-starter.
SALARY GRADE AND CLASSIFICATIONS: non-exempt
RESPONSIBLE TO: Director of Care Coordination
SUPPORTIVE/CHARGE RESPONSIBILITIES:
- Works in harmony with others exhibiting a positive attitude and a commitment to the basic mission of the health center.
- Track assigned referrals to ensure patient access and support patients in overcoming barriers to care.
- Provide basic instructions/health education to clarify provider instructions, procedures and referral needs.
- Assist patient with preventative care and help find affordable places to receive the care. If a patient does not have insurance, provide information about applicable services and programs they may qualify for.
- Assist patients with completing applications for patient assistance programs such as Oklahoma Project Woman, Rx for Oklahoma, Vision USA, etc.
- Monitor Chronic Care Management (CCM), which includes: documenting patient consent to participate, time spent providing non-face-to-face care, and care plan updates.
- Acknowledges that the Case Manager role is constantly evolving to meet the needs of patients. Working daily Share file to create incoming referrals for imaging, emergency visits and/or hospital stays.
