About the Role
As a Physical Therapist in Union Grove, you will help residents improve mobility, regain function, and maintain independence. You’ll develop individualized treatment plans, collaborate with a skilled interdisciplinary team, and deliver meaningful care that changes lives.
Key Responsibilities

Conduct comprehensive assessments of residents’ physical function and mobility

Develop and implement individualized treatment plans

Provide skilled physical therapy interventions

Educate patients and caregivers on mobility strategies and fall prevention

Recommend appropriate assistive devices and modifications

Maintain accurate, timely clinical documentation
-
Collaborate with nursing and therapy colleagues to ensure holistic care
Qualifications
-
Graduate of an accredited program with a bachelor’s, master’s, or doctoral degree in Physical Therapy
-
Licensed or eligible for licensure in the State of Wisconsin
-
Strong clinical, communication, and organizational skills

A passion for working with older adults in a skilled nursing or rehabilitation setting

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
