What are the responsibilities and job description for the Compassionate Care Provider position at Guy Care Service?
Current shift schedules available:
- Wednesday, Thursday & Friday (4:00pm - 8:00pm)
- Additional 8 hours per week - days not yet determined
Total of 20 hours per week to start, with the option to add another client if desired.
We have additional immediate shifts available in Nashville and Antioch.
More shifts are available once a new hire is onboarded if desired!
Duties
- Assist adults (aged 21 ) with disabilities with daily living activities, including personal hygiene, dressing, and grooming.
- Provide companionship and emotional support to members, fostering a positive environment.
- Monitor client health by observing changes in condition and reporting any concerns to healthcare professionals.
- Maintains accurate and current documentation records via PHS (Practical Health System) and/or CareBridge system.
Qualifications
- Must be at least (18) years of age.
- High school diploma or GED required.
- Experience with CareBridge, InfoServ or a similar system is preferred.
- Knowledge of DDA, DIDD, DCS or similar government programs is preferred.
