JOB SUMMARY:   Working under the guidelines of the Center of Excellence, this position provides formal case management services to eligible opioid dependent individuals to promote healthy outcomes.

ESSENTIAL FUNCTIONS: 

  1. Provide service coordination that includes case management, care coordination, client advocacy, and a framework of action for the client to achieve specified goals.
  2. Meets regularly with clients to implement housing plan, stabilize clients’ own home and then develop life plan goals. Encourage progress towards these goals by developing budget, employability, family supports and counseling, and other community resource referral as quickly as possible.
  3. Maintains relationships with landlords and community resources to connect clients with housing opportunity.
  4. Document service coordination activities throughout the continuum of care.
  5. Assess Service Plan and in consultation with the client, make appropriate changes to the Service Plan to ensure progress toward treatment and/or recovery goals.
  6. Arrange for advocacy services around issues of housing, client or family involvement with court system or other services relevant to the client’s well-being. 
  7.  Maintain updated list of local agency contacts and list of key documents required for each housing and service offering to help expedite process.
  8. Maintain ongoing contact with client and involved support systems to ensure adherence to the treatment plan.
  9. Provide transportation support to minimize barriers to continuum of care. 
  10. Establish and maintain relations with civic groups, agencies, and other professionals, governmental entities, and the community-at-large to ensure appropriate referrals, identify service gaps, expand community resources, and help to address unmet needs.  
  11. Apply confidentiality regulations appropriately.
  12. Provide reports and paperwork as required in a timely manner.
  13. Attend all meetings as required.
  14. Attend all core trainings for drug and alcohol case management and addiction trainings that pertain to the role of housing case manager.
  15. Attend community outreach events as needed. 
  16. Maintain identified certifications and minimal training hours annually 

 

Minimum Education/Certifications: Bachelor’s degree preferred.  In lieu of Bachelor’s, minimum 10 years of experience required in addition to enrolling in and continuing education credits towards a bachelor’s degree in Social Work, Case Management, Social Science, Psychology or Addictions or related field (at least part time) within the first 6 months of employment required.     

 

Minimum Work Experience:   Minimum of one year’s case management experience preferred.

Other Requirements: Reliable transportation to travel throughout the service area.
