We are looking for an experienced MDS Coordinator to join our team and provide exceptional care to our residents. The ideal candidate will have a background in geriatrics and nursing home care, and be able to effectively coordinate the Minimum Data Set (MDS) process for our residents. The successful candidate will also be responsible for ensuring that all MDS forms are completed accurately and on time, as well as providing additional support and guidance to other staff members.
Responsibilities:
- Monitor and evaluate the quality of care provided to residents.
- Coordinate the activities of the interdisciplinary team in the provision of care.
- Ensure compliance with federal, state, and local regulations and standards.
- Develop and implement policies and procedures related to the care of residents.
- Provide education and training to staff members on the proper care of residents.
- Assist in the development of individualized plans of care for each resident.
- Monitor resident progress and document changes in condition.
Job Type: Full-time
Pay: $28.00 - $34.00 per hour
