What are the responsibilities and job description for the Shared Living Provider position at Hands of Heartland?
We are looking for compassionate, dependable people who would like to share their home with a person with developmental disabilities. Hands of Heartland has been supporting people with developmental disabilities since 1997.
Responsibilities
Providing supports to a person with a developmental disability to ensure his/her wants and needs are met in a safe environment in compliance with the Nebraska Medicaid waiver program standards. Examples include:
- Implementing programs in accordance with the person's plan
- Ensuring the person completes activities of daily living (grooming, eating, bathing, dressing, and personal hygiene) and provide any assistance if needed
- Assisting with meal planning and preparation
- Providing reliable and timely transportation to day services and all health service-related appointments, work, etc.
- Assisting with ambulation and mobility as needed
- Reinforcing counseling and therapy activities
- Assisting with medications and the performance of tasks delegated by a Registered Nurse
- Providing supervision to ensure the person's safety and security
- Facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behaviors
- Other services designated by the person's plan
- Completing and maintaining required documentation on a web-based system
Qualifications
- Valid driver's license
- Current automobile insurance
- Ability to pass registry and background checks
- Ability to be certified and maintain certification in verbal and physical intervention, First Aid/CPR, and Nebraska Medication Aide
- Prefer at least one year of experience in intellectual/developmental disabilities or caring for a person in a healthcare setting
