What are the responsibilities and job description for the Case Manager position at Harbor Brand?
Harbor is currently looking to add a Case Manager in the Toledo area to serve clients with substance abuse and behavior health problems.
Position is full-time (40 hours per week.
Education/Experience/Other Requirements:
- Associate’s Degree in Counseling/Social Work/ Related Field required; Bachelor’s degree in human service field or equivalent experience preferred.
- Knowledge of 12-step groups of recovery.
- Must have valid driver’s license, acceptable driving record, and be deemed insurable
through CCHB’s insurance carrier.
o For residential programs; Must be at least 23 years of age, have reliable transportation, Must remain up-to-date on annual Defensive Driving for 15-Passenger Vans training requirement. - Chemical Dependency licensure required within 90 days of employment.
o For Inpatient Withdrawal Management Program, CDCA is required at time of
hire. - When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months.
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
Essential Job Competencies/Primary Duties:
1. Schedule and meet with each client as needed, develops case management treatment plan, and schedules aftercare appointments.
2. Organizes, schedules and coordinates all case management activities including transportation to appointments.
3. Develops and maintains an internal case management data system that is readily accessible to staff.
4. Completes all appropriate paperwork for client charts.
5. Networks with outside agencies to ensure individual client needs and goals are met.
6. Presents workshops to other agencies on women’s specific recovery issues.
7. Establish and maintain alumni support groups.
8. Participates in training and development programs to enhance knowledge, skills and personal service network.
