Harbor is seeking a provider with lived experience who delivers professional services to program participants and their families participating in the Benefits Bridge program. This position offers support to individuals and families navigating benefits cliffs, provides job support, and refers for services and community programs as identified on individual service plan.

Position is full-time, 40 hours per week.

Education/Experience/Other Requirements

  • High School Diploma or GED required, associates degree or higher preferred.
  • Present and/or past experience receiving public assistance (e.g., childcare assistance, SNAP benefits, OWF benefits, etc.). 
  • Must obtain EMPath Certificate (Economic Mobility Pathways) within 120 days of hire.
  • Must possess the skills necessary to help clients achieve and maintain their highest level of functioning by striving for successful community living.
  • Must have the ability to offer a unique perspective of receiving public assistance because of his/her own experiences and relate his/her journey towards self-sufficiency.
  • Must be proficient and accurate in computer use, including Microsoft Word.
  • EMPath Certificate (Economic Mobility Pathways) must maintain current continuing education and requirements for renewal for their certificate. 
  • Must be willing to work nights and weekends when needed.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance policy.

Essential Job Competencies/Primary Duties:

  1. Uses experiences with public assistance and services in assisting and supporting participants on an ongoing basis.
  2. Completes comprehensive assessments, bridge scores, and develops service plans with participants.
  3. Refers and supports participants in attending and completing financial literacy training and trainings/classes that support successful completion of bridge program.
  4. Provides direct services to participants, maintains weekly contact, identifies needed support services, monitors goal achievement for earning incentives, and identifies educational/job training needs.
  5. Assists team, participants, and staff members in completing documentation necessary to determine eligibility for community resources.
  6. Acts as advocate for participant to secure needed services.
  7. Attends and may lead Care Team meetings, provides updates to care team on participants progress, participant successes, and barriers to program completion.
  8. Links and supports participants in completing financial literacy training and works cooperatively with financial opportunity centers.

ONGOING INTERVENTIONS

  1. Provides support, education and consultation to participants and families, and/or significant others.
  2. Provides supportive interventions as noted in the service plan to support participant in transitioning to greater self-sufficiency and progressing in identified career goals.
  3. Provides education on job search, job success, budgeting, problem solving to participants, families and/or significant others.
  4. Assures appropriate communication with team leader and team members responding to participant calls and requests.
  5. Assists in participants acquisition of job searching and employment skills.

COLLABORATION

  1. Assists the participant and family in increasing social support skills and networks.
  2. Coordinates indicated evaluations and assessments by internal and/or external providers as appropriate to the position.
  3. Coordinates and monitors all services identified in the ISP as appropriate to the position.
  4. Provides assistance in gaining access to essential community resources. Locates and/or establishes self-help and support groups for participants, families, and/or significant others.

SPECIAL WORK CONDITIONS

  1. Maintains participant confidentiality and adherence to HIPAA requirements.
  2. Participates in required agency and departmental meetings and staff trainings.
  3. Mobility Mentor is required to work closely and cooperatively with a variety of service providers in the community as well as within Harbor.
  4. Ability to provide culturally competent services. Capacity to seek consultation and supervision, when complexity of situation is greater than employee’s knowledge base.
  5. The mobility mentor is a highly visible position within the community and in the agency. The person in this position must have solid knowledge of agency services and must know how to secure services if not available in the agency.  This person must subscribe to Harbor’s mission and values.  Competency regarding cultural/sociological/racial issues is essential.
  6. The Mobility Mentor position requires working with participants in their natural environment. The position is community based and may require travel within the community.
  7. Types documentation in the format appropriate and acceptable to Harbor.
  8. Keeps current with trends and developments related to essential job competencies and demonstrates continued growth.
