About the Role
As the Intake & Referral Coordinator, you’ll be the first point of contact for new referrals, ensuring a smooth and welcoming start to care for our patients. Your role will be critical in facilitating the transition from referral to admission by collaborating with physicians, patients, and internal clinical teams.
Key Responsibilities
- Manage incoming referrals and verify patient eligibility for services.
- Collaborate with referral sources, including physicians, hospitals, and community partners.
- Coordinate patient admissions with our clinical team to ensure timely care.
- Maintain accurate and organized records in compliance with healthcare regulations.
- Provide exceptional customer service to patients, families, and referral partners.
What We’re Looking For
- Experience in healthcare intake, referral coordination, or a related field (home health experience preferred)
- Strong organizational and communication skills.
- Ability to handle multiple tasks with a high degree of accuracy and efficiency.
- Proficiency in healthcare software and documentation systems.
