Position Purpose:
Provide person-centered services to the people we support in all aspects of their lives.
Provide direct supervision and support to DS Direct Support Professionals (DSP) who assist the people we support in navigating their communities in a way that promotes growth and independence.
Develop and maintain community relationships and identify resources within various community settings to increase community integration. Provide ongoing assessment of skill acquisition and support requirements that individuals receiving supports need in order to be successful within their communities. Provide ongoing training and mentorship as needed, to supervisees, in order to increase their skills and abilities to support the people we serve.
Principal Responsibilities and Duties:
- Hire, train and supervise identified DSPs within office.
- Supervise approximately 8 DSPs
- Provide person-centered community supports to identified clients
- Review chart notes for compliance and detail, including Behavior tracking notes
- Ensure staff is trained on all mandatory paperwork for DAIL and HCRS and that paperwork is completed timely.
- Developing outreach community-based and virtual activities
- Work with DSP Manager to develop and implement structured, client-driven, skill-based small group services for interested clients
- Complete all mandatory trainings and ensure all staff are up to date with mandatory trainings.
- Attend supervision and ongoing Community Support Meetings as deemed appropriate by the office Area Manager, DS Assistant Director and/or DS Director.
- Identify, develop and maintain meaningful community partnerships and activities for clients to reach their goals, dreams and objectives.
- Provide on-site supervision to DSPs while in the community, as necessary.
- Attend all Team Leader meetings with DSP Manager(s).
- Maintain positive relationships with HCRS stakeholders and community members.
- Provide scheduling guidance to Senior DSP and support in response to unexpected absences.
- Implement plans as part of service delivery and support DSPs to do the following. Examples of plans to implement include, but are not limited to: ISAs, behavior plans, communication plans, etc.
Scope of Position:
Oversee the office’s DS client community support needs. Maintain compliance with regulatory bodies and foster open and frequent communication with all Service Coordinator’s and other vital team members to ensure a cohesive outcome for the people receiving supports from HCRS.

- Understanding of common developmental disability and mental health diagnoses and how related challenges typically manifest in one’s day-to-day experiences
Relationships:
- To be a team member with Service Coordinators, DSP Managers, Area Managers, Behavioral Specialist, parents and or guardian, DS Nurses, community partners and liaisons.
- This position supervises approximately eight DSPs
- This position is supervised by the office DSP Manager
Qualification Statements
Minimum:
- Bachelor’s or Associates degree and/or working on degree and at least three years of experience in the field. Five years of related experience can serve in lieu of a degree.
- At least three years of experience working with individuals with developmental disabilities.
- Proficiency with Microsoft Word, Excel and PowerPoint.
Preferred:
- Bachelor’s degree with Human Service background
- Previous supervisory and/or mentoring experience
- Five years of experience working within the Human Service field
- Five years of experience working with individuals with developmental disabilities.
- Proficiency with Credible and scheduling software.
