What are the responsibilities and job description for the Manager, Case Management position at HCTHealthcare?
HCT is seeking an experienced nursing professional to serve as interim Manager of Case Management for a 200 bed hospital in California. The responsibilities include:
- Providing oversight, direction and leadership of the Care Coordination functions including utilization management, discharge planning, case management, length of stay management, and the appeals/denials process.
- Collaborating among clinicians, including social services, discharge planning, utilization review, and patient throughput in regard to assessment, planning, implementation, coordination, monitoring and evaluating the options and services required to meet the patient's complex medical discharge planning and transitions of care needs.
Requirements:
- Education: Bachelor's degree required.
- Experience: Minimum of three years of experience in managing care coordination and discharge planning in an acute care setting is required.
- License: RN CA license is required.
- Certifications: ACM, CCM or other relevant certifications preferred.
- EMR: Cerner
