Description
The Comprehensive Health Assessment Writer will assist the BCDSS permanency staff by researching, compiling, and documenting the background, education, and health histories of all children entering Baltimore City Department of Social Services (BCDSS) Foster Care.
The primary responsibilities for this position are:

Research the family history, prior agency history, education, and disability information for all children entering BCDSS Foster Care.

Attend either in person or by phone all Initial Case Plan Family Involvement Meetings for children entering BCDSS Foster Care.

Compile all researched information and all current medical and mental health information into a comprehensive, well written, easily read document for sharing with all necessary parties, within 60 days following the entry of a child into BCDSS Foster Care.

Utilize both MATCH Database and BCDSS Database for research and entry of information.

Answer the telephones and assist callers, as necessary
Qualifications
Minimum Qualifications:

Bachelor’s degree in human services or health related field

Experience in reviewing and summarizing medical and mental health documentation

Ability to effectively gather historical and educational information via telephone in a professional manner

Strong command of English grammar and language

Well developed interpersonal & communication skills

Proficient in MS Word, Excel, Outlook , Google mail and docs

Professional appearance and demeanor

Familiarity with database systems

Customer service skills
-
Ability to answer a multi-line telephone system and direct calls to the appropriate person

Familiarity with community based resources
Physical Requirements:
