Job Description: Job Description Pay: $23.00-$25.00 per hour, $2,500 Sign on Bonus EXPERIENCE: Minimum of one year (1) year experience providing care to the elderly and/or disabled in the home or a health care setting. EDUCATION: Bachelor’s degree plus one year professional experience with aging or disabled population obtained before or after receipt of degree, performing duties which encompass the core function of case management including but not limited to assessment, planning, implementation, monitoring and reassessment. Paid professional experience may include, but not limited to: CNA, CMA, CHHA, and PCA experience. LICENSURE: RN or LPN license in state of business SKILLS: Ability to motivate others in a positive manner Excellent verbal and written communication skills Professional interpersonal skills Highly organized Excellent computer skills Able to support and foster a team philosophy focused on quality service Able to monitor productivity and efficiency EDUCATION: Bachelor’s degree in Healthcare Management. EXPERIENCE: Minimum five (5) years performing duties which encompass the core function of case management. One year of professional experience with aging or disabled population obtained before or after receipt of degree. Paid professional experience may include, but not limited to CNA, CMA, CHHA, and PCA experience. Supervisory experience is preferred in a home health or hospice environment. CERTIFICATION/LICENSURE Case Manager certification and a current registered nurse (RN) license in the state of licensure or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
