- Hospice Care Social Worker Job Description :
- Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies
- Facilitate psychoeducational support groups
- Provide in-services to other service providers and organizations
- Lead community education workshops
- Plans for discharge, coordinates care, and helps clients navigate systems; identifying and linking clients with resources
- Facilitates advance care planning and lifespan planning
- Intervenes in crises
- Mediates conflicts within families, clients, and the interdisciplinary team, and between service organizations
- Participates in interdisciplinary team meetings, care planning, and ethics consultations
- Advocates on behalf of the patient and family
- Document social work activities
- Social Worker Requirements :
Licensed Social Worker in Missouri or Illinois required.
60,000 to 68,000 Depending on Experience
