HealthWorks Medical, LLC is looking for a self-motivated and team-oriented Nurse Case Manager to join our team. Hours are typically Monday through Friday 8:00 am until 5:00 pm (with telework options). Some early morning hours and some late afternoon short-notice overtime is also a possibility.

Job Description Report:

• Acts as patient advocate in delivery and coordination of patient care.
• Provides case management to a group of patients providing specific assessments according to protocols and procedures
• Clinical competence in procedural skills and treatments
• Provides efficient and effective care with the guidance of the physician during the clinic sessions, attends the clinics where scheduled and is dependable.
• Able to manage a number of patients at the same time with high energy and organization
• Collaborates with other health care providers and Medical Director regarding clinic and patient needs.

• Ability to work independently and make decisions
• Ability to effectively communicate with all levels of management and staff
• Ability to organize, perform and prioritize multiple functions and tasks
• Ability to organize, attend and participate in staff development, training, and wellness programs
• Ability to travel between clinic locations

Minimum Education- Associates Degree or; one or two years related experience and/or training; or equivalent combination of education and experience.

Required Licenses [Kentucky, United States] RN or LPN

• Registered Nurse with a current KY license or valid compact state license OR
• LPN with a current KY license or valid compact state license
• BLS certification - American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.

Minimum Work Experience:1 Year of Case Management Experience (preferred)
