Job Description
Seeking compassionate and friendly Licensed Clinical Social Worker to provide integrated behavioral health services to patients receiving care at Heart City Health.
Qualifications
- LCSW required. Experience (2 years or more preferred) serving clients with behavioral disorders, psychological illness, and/or substance use disorders. Strong clinical skills with experience in outpatient setting required.
- Demonstrated knowledge of behavioral disorders, psychological illness, and substance use disorders including, motivational interviewing engagement, de-escalating strategies, and knowledge and skill in the use of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Must have strong commitment to client rights.

Responsibilities
As part of the integrated health model of Heart City Health, this position provides integrated behavioral health services to patients currently receiving primary care services at HCH. The Licensed Clinical Social Worker works as an interface for patients accessing services such as individual, peer/family counseling and support, screening, assessment, brief treatment, consultative services, and education. This position works directly with primary care providers to strengthen relationships between behavioral health and primary care services at Heart City Health.
