Family Nurse Practitioner

★ Medical Coverage

• Your Immediate Family - you, spouse and children living in the home or in college will be set up as patients and we will accept what is paid insurance.
• This will include anything we provide other than labs.

■ You will be expected to pay 50% of the amount billed by us for the lab.

• We Offer BCBS Health Insurance, Guardian Dental and Vision and Supplemental

■ Your BCBS Monthly Premium Will Be Paid at 50% and Start Day 1
■ If You Add Spouse or Children then You are Responsible for 100% Added
■ You are Responsible for 100% Dental, Vision and Supplemental

★ Compensation

• Base Pay will be $100,000 Annually.
• Managing Partner and Earn 10% Equity in the Company
• There is a $12,000 Annual Bonus Available.

■ It is based on $1,000 Monthly
■ Paid the 2nd Friday of the following month as a separate check.

30% All Charts closed within 48 Hours (10%) with Maximum Billing (10%) and All Labs
Reviewed within 48 Hours (10%)

10% Enjoying Your Career & Having Fun at Work (5%) with No Legitimate Patient
Complaints (5%)

30% Seeing a Minimum of 150 Patients Weekly. If there is a Day without 30 Possible
Patients then the Weekly Amount will be Adjusted for what is possible (30%)

10% Enjoying Your Life & Spending Quality Time with Family While Not at Work (10%)

20% Ensure Your Nurse is Returning All Voicemails & Messages Daily and Calling all
Patients seen to Ensure they are Better and Got Medications (10%) Keeping Trash
Emptied, Rooms Stocked, Sweeping, Mopping and Cleaning (10%)

★ You will be provided with an annual evaluation in December of each year.

• The evaluation will be up to 5% of an increase in pay. The percentages will be totaled and multiplied by 5% to get a total percent and multiplied by pay.
• The evaluation will carry the following percentages:

■ 50% Monthly Bonus Evaluations
10% Timeliness and Attendance
10% Rapport with Patients
10% Ability to Work as a Team Player & Maintain Positive Attitude
10% Help Promote the Business with Website, Video Creation, Social Media, Etc.
10% Ensure Your Ancillary Staff Does Not Incur Unnecessary Overtime

★ Schedule Will Be Monday Thru Friday 7:30 AM until 5:30 PM.

• You will need to see a minimum of 150 patients each week (30 Patients/ Day).
• You will be Salary
• You will take a 1 Hour Lunch
• You will be given 30 Minutes each day for Lab Reviews.

■ Labs must be reviewed and medications ordered within 48 hours.

• You will be given 1 Hour each day for Charting.

■ Charts must be charted on and closed within 48 hours.

★ You are expected to wear a Provider’s White Lab Coat and scrubs while seeing patients.

• Please go to J&D Uniforms in Southaven. They have our Logo for Embroidery.
• We will Provide You with one White Coat each year.

★ You will Receive 2 Weeks Vacation.

• Each week will be paid at 40 hours.
• This time will be available after 1 year of employment.
• Must be used within 12 months or you will Forfeit the Time.

★ You will receive 3 days of sick time per year.

• All Sick Time Must Be Used within 12 Months. In the event you depart from the Company or Do Not Use the Time within 12 Months, you will forfeit the time.

★ You will receive 7 Paid Holidays that the Clinic Closes.

• They include New Year’s Day, Good Friday, Memorial Day, The Fourth of July, Labor Day, Thanksgiving Day and Christmas Day.
• There is the option to close on New Year’s Eve, Black Friday and Christmas Eve. This is up to the Provider but these days will not be paid.

★ Heartland Family Medical will reimburse you for the following:

• DEA Licensure
• CPR Certification
• MS BON RN and FNP Licensure
• Medical Malpractice
• Provide a receipt and we will pay you for these items.

★ Continuing Education

• You will be provided with 3 Paid Days Off Each Year
• Up to $1,000 annually to use for event registration, travel and meals.
• We will need receipts for accounting.

★ Prepare Monthly Paperwork for Collaborating Physician

• PMP Print Out
• List of 20 Patients

★ Provide Office Manager with Any Paperwork Needing Collaborating Physician’s
Signature so it can be signed weekly
