- Request and obtain healthcare visit summaries and other medical records from community providers for foster care youths managed by Care Coordination program, including historical medical records as applicable
- Upload and file medical records and other required documents in NextGen charts
- Identify, download and share NextGen documents as requested, such as for adoption packets, audits and investigations, etc.
- Track through NextGen the foster care youths annual required healthcare assessments, medical/dental/behavioral health appointments, and other related encounters/services
- Create NextGen system reports to meet departments needs
- Coordinate with Care Coordination team, Case Planning, Childrens Health Homes (CHH), 29-I Clinic and other HSVS staff, as well as with community partners, to help ensure that required NextGen medical record documentation is complete and accurate, in compliance with current City/State foster care healthcare standards
- Assist departmental staff with the completion of needed actions in NextGen, including referrals to HSVSs Article 31 behavioral health clinic, CHH Warm Hand-offs, Comprehensive Treatment Plans, data share reconciliations, etc.
- Support HSVS EHR System Administrator in training department staff and troubleshooting issues in NextGen EHR
- Engage in ongoing review of departmental policies, procedures and practices to make recommendations for quality improvement
- Perform other tasks as assigned within the reasonable scope of duties of the position
- A Bachelors degree in health or social services, or equivalent
- One or more years experience managing medical records in a healthcare environment and/or the child welfare system
- Solid working knowledge of NextGen EHR or similar software system
- Advanced understanding of medical terminology and administration processes
- Experience training staff a plus
- Strong organization skills and attention to detail
- Proficiency in Microsoft Office, including Outlook and Excel
- Excellent written, oral and interpersonal communication skills, and be a collaborative team player
