Intake
Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
The Assertive Community Treatment and Community Programs Coordinator oversees the ACT and community programs with locations in Westland, Lincoln Park and Taylor.
REQUIREMENTS:
- Bachelor's Degree from an accredited college or university, specializing in psychology, social work, counseling, or other related human services field
- Knowledge and experience of/with ACT preferred
- Five years of progressively more responsible experience in a related mental health and substance abuse services field
- One year or more providing direct treatment services to adults in a behavioral health inpatient or outpatient setting
- Basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population
RESPONSIBILITIES (summary):
Intake
- Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement.
- Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV risk, education, and vocational training.
- Educate patient and, if applicable, patient guardian with respect to admission information packet.
- Demonstrate knowledge of patient payor source during intake process.
- Assessment, as required, within seven days of initial interview.
- Schedule appointment for psychiatric evaluation.
Treatment Planning and Progress Reviews
- Complete individualized Treatment Plan for each patient.
- Treatment Plan goals and objectives demonstrate that the plan for treatment was developed with the patient.
- Utilize the patient’s Person-Centered Questionnaire in the development of the treatment plan and support person participation.
- Treatment Plan problem areas, goals, and objectives address the patient’s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment.
- Complete Treatment Plan Reviews (TPR).
- Update Treatment Plan via TPR following changes in clinical status.
Discharge Process
- Document termination of services on the Treatment Plan and on a Progress Note when patient continues to receive other clinical services from HHI.
- Document on Treatment Plan criteria and anticipated date for planned discharge.
- Discharge patients with no contact for 45 days.
- Complete Discharge Summary within 21 days of discharge date.
- Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges.
Ongoing
- Provide individual supportive, solution-focused interventions, not less than twice every 30 days, to all patients on caseload.
- Coordinate appointments with psychiatrist and other HHI service providers, as applicable.
- Provide linking and coordination of community-based services.
- Assist patients and/or adjunctive service providers with various case management tasks, including but not limited to, medication compliance, referrals, and linkage to resources.
- Transport patients as needed to community-based supportive, adjunctive services.
- Meet with patient in hospital within two (2) working days of notification of hospitalization.
- Meet and discuss aftercare with inpatient social worker at the time of the initial face-to-face contact with the patient.
- Assist with problem solving and behavior management concerns in regards to patient maladaptive behaviors or decompensation.
- Complete a progress note for all treatment services.
- Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or clinic supervisor immediately.
- Consult with clinic psychiatrist within five days of the identification on a non-emergent medical condition.
