Description
Under the supervision of the Director of Utilization and Quality Management, this position is responsible for the delivery of clinical services and community supports to persons with severe and persistent mental illness, as defined by the state authority, The Health and Human Services Commission (HHSC). Services are to be provided according to the Texas Resilience and Recovery Model, and must conform to the Performance Contract, Community Service Standards, Medicaid standards, and Helen Farabee Centers (HFC) policy and procedures.
Responsibilities include, but are not limited to:
The incumbent is expected to assist individuals in making informed choices, educate them on community resources, and assist with accessing services. The incumbent is to perform intakes which determines diagnosis/diagnoses, performs uniform assessment, gathers social history information, and determines initial and on-going eligibility for services. The incumbent must provide face-to-face assessment and crisis intervention services to any individual requesting help or presenting in crisis at any time. Teamwork, initiative, and positive example in work habits are expected at all times. The incumbent is to provides consultation and support to fellow staff as needed. This position is designed for someone with compassion, competency, and proficiency for working with persons who have severe and persistent mental illness, and other social determinants of health. Helen Farabee Centers considers this work as a clinical specialty in which we take great pride. The incumbent should have knowledge and skills in assessments, diagnostics, treatment planning, and community support services for children, adults, and geriatric persons with mental illness, as well as knowledge of psychotherapeutic medications and basic casework skills. This position also provides counseling services to individuals who are diagnosed with depression and/or anxiety using Cognitive Behavioral Therapy (CBT) and PTSD using Cognitive Processing Therapy. Certification in each protocol is required. HFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all individuals served and staff. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employer’s discretion. Work location and hours could change based on program needs.
Qualifications
Education, Training, and Experience
Master’s degree from an accredited college or university with major in social, behavioral or human services field. Completion of work toward Licensure and achievement of appropriate professional certifications (i.e. LCSW, LPC, LMFT ) required. Experience with Severe and Persistent Mental illness is preferred but not required. Must have valid driver’s license and be insurable to drive Center vehicles. Must pass all Center mandated trainings.
Knowledge, Skills, and Abilities
- Ability to read and proof correspondence, reports and computer printouts manually, etc.
- Good spelling and grammar skills.
- Ability to write progress notes according to standard, assessments, brief mental status exams, memos, letters, various complex reports and miscellaneous other documents with acceptable spelling and grammar.
- Ability to relate verbal instructions, communicate in a courteous and effective fashion with co-workers, individuals served and families, agencies and the public.
- Display a customer service orientation; at all times treating individuals as equals and partners.
- Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office), SmartCare, familiarity with CARE.
Salary : $60,528 - $62,571
