- Collaborate with the Population Health Manager to design, implement, and execute population level initiatives to improve quality, efficiency, and use of healthcare services on behalf of HNJH Community Clinics and its patients.
- Accountable for coordinating, facilitating, monitoring, and evaluating patient population to ensure quality of care that is consistent with evidence based guidelines.
- Work closely with Primary Care Practices, patients, and insurance carrier quality programs and initiatives to improve care coordination services, patient outreach, staff education that supports quality outcomes, smooth transitions, and coordination or care across the health care continuum for the patients served.
- RN or LPN License for the State of Michigan
- BLS required
- ACLS & PALS preferred or within 6 months of hire
