Home Health Aide

Position Overview:

An individual that provides a broad range of non-medical, supportive services, including bathing, nail and skin care, oral hygiene, toileting, safe transfers, range of motion, nutrition and fluid intake, medication reminders, and any other task appropriate according to the patient's care plan.

Responsibilities:

• Follows a client's written Plan of Care and carries out duties as assigned by the supervisor
• Communicate professionally, including making brief presentations to patient and caregivers
• Observe and report any safety hazards found in the client's home or any significant observations regarding the client
• Observe, report, and document patient status and the care or service furnished
• Follow basic infection control procedures and universal precautions
• Report abnormal findings in client's condition to the supervisor
• Report client/family complaints to the agency
• Maintain client confidentiality in accordance with HIPAA requirements
• Accurately follow written and oral instructions
• Attend staff meetings, case conferences, team meetings and educational in-services as required
• Other duties as assigned
• Possess the physical abilities to conduct the following:
• Frequently sit, stand, walk, bend or squat
• Occasionally crawl, climb, or reach above shoulder level o Occasionally lift up to and over 50 lbs
• Continuously see, hear, speak and use both hands

Required Qualifications:

• At least 18 years of age
• Able to read, write and follow directions
• Complete and maintain current CPR certification
• Complete PPD or Chest x-ray if applicable
• Successfully complete Help at Home required training, pre-service, competency evaluation, and orientation
• Work positively with clients, family, staff and external agencies
• Demonstrate compassion toward all persons regardless of age or any physical or mental disability
• If transporting clients, provide proof of active driver's license and up to date auto insurance

Preferred Education and Experience:

• High school graduate, or completion of the equivalent education
• At least 6 months of experience providing care in a similar role
