Job Summary
The Care Manager assumes responsibility and accountability for assisting in the planning, organization, supervision, evaluation, delivery, and reporting of all care and nursing services in the assisted living and memory care facilities. The Care Manager will be assigned a caseload in the facilities and shall assist the Community Manager in ensuring the direct care team is compliant with federal, state, and local regulations and implementation of the community’s programs and policies. Follows all mandated disease control policies. Adheres to the budget policies and participates in census building. Help strategize with new processes to adhere to best practice standards and quality measures. Implements and follows through of new policies, procedures, programs, and processes as directed by the Director of Clinical Services.
Major Responsibilities
1. Able to perform and complete proficiently in all clinical competency areas including but not limited to: training, charting/documentation, direct patient care, medication administration/management, patient rights, infection control, safety, and sanitation.
2. Collaborate with physicians, pharmacists, and other clinical providers
to coordinate care and services for the residents to ensure residents’
treatment requirements are met.
3. Manage caseload to complete Admission Service Care Plans both
documentation and 1:1 resident evaluation to promote continuity of care and
quality health outcomes.
4. Complete/Report
the results of the Admission Tool (LOCAT) within 48-72 hours of request. Collaborate with Sales partners to determine community
capability and assess potential residents’ appropriateness for move- in.
5. Complete
resident admission clinical assessments evaluating for pain, skin, and health.
6. Oversee
resident charts for accuracy including chart auditing. Obtain Medication
Records, TB test, Durable Power of Attorney, and end of life choices documents
are placed in the chart.
7.Complete
Level of Care (LOC) evaluations as scheduled for appropriate staffing for
patent care.
8. Utilize
Clinical Rounding Tool as the structured,
and goal-oriented rounding strategy aimed at improving rounding efficiency
while maintaining the foundational aspects of patient care and clinical
education critical to resident care delivery and Interdisciplinary Team collaboration.
9. Complete
the reporting requirements when there is a change in resident condition to notify
the Interdisciplinary Team and Supervisors and follow the Family Communication
Policy.
10. Assist with admissions, discharges, and residents’ transitions throughout
the continuum .
Practice
infection control policies and procedures.
11.Monitor
Medication effectiveness, adverse effects, and report these to Physician, Med
Techs and Care Assistants.
Coordinate
end of month Medication Administration Record change over.
12. Follow
the medication disposal policy, pharmacy protocol for medication reviews, attend
training.
Required Certifications, Licenses, and Registrations
1. Licensed RN or LPN in the State of Michigan required for Nurse Care Manager.
2. Current CPR certification.
