What are the responsibilities and job description for the Case Manager position at HfH Supportive Housing?
PRIMARY OBJECTIVE/SCOPE:
To provide case management services for previously homeless residents by working to mobilize the required health care and supportive services for each participant to remain stably housed.
WORK PERFORMED
- Work with the local Coordinated Entry System to qualify referrals for a low income housing project with required set-asides.
- Process requests for services by conducting intake and a variety of assessments.
- Coordinate various aspects of Case Management services; including referrals, program planning, monitoring, assessment, and evaluation of resident needs and services
- Serve as an advocate for residents internally and external.
- Document case notes after each interaction with a resident/family
- Enter resident data into the agency’s data entry system and run interagency repots.
- Coordinate community outreach projects to help residents reach economic self-sufficiency.
- Provide residents with supportive services.
- Collaboratively work with on-site Property Management team to ensure an integrated support system for each resident.
- Access community resources for the benefit of residents and their family members.
- Travel to other social service agencies, and other destinations required to perform daily tasks, participate in staff meetings, case conferencing, community meetings, etc.
- Adhere to all privacy and confidentiality policies and procedures.
- Serve in the role of a mandated reporter as instructed by the Department of Children and Families (DCF).
- Complete appropriate training, as assigned
- Other duties as assigned by the Social Services Manager
SKILLS/ABILITIES DESIRED:
- College degree/at least 5 years of social service experience
- Ability to pass a Level II background screening
- Ability to work with MS Office applications and strong typing skills preferred
- Experience in Medicaid billing preferred
- Strong communication skills, both written and verbal are required
