Job Title: Qualified Mental Health Professional-Community Services (QMHP-CS)

Job Summary:
Hidden Roots Mental Health is seeking an experienced and compassionate Qualified Mental Health Professional-Community Services (QMHP-CS) to join our team in Texas. The QMHP-CS will provide individualized support, case management, and therapeutic services to individuals in the community. The goal is to help clients achieve and maintain stability, independence, and improved mental health outcomes within their communities.
Targeted Case Management Services:
Provide person-centered, goal-driven case management for individuals with mental health needs. This includes assessing client needs, developing individualized service plans, coordinating access to healthcare, social, and community services, and monitoring progress to ensure positive outcomes. Case managers will collaborate with clients, families, and service providers to address barriers and connect clients to the resources necessary for achieving stability and improving their quality of life.
Duties and Responsibilities:
- Service Delivery:
- Implement and monitor Individualized Service Plans (ISP) tailored to client needs.
- Provide skills training, psychosocial rehabilitation, and support for activities of daily living.
- Facilitate family relationship building and parenting skills training.
- Provide case management services, including linkage to community resources and coordination of care.
- Conduct crisis intervention and stabilization services as needed.
- Documentation and Compliance:
- Complete and submit all required assessments, progress notes, service plans, and reports accurately and on time.
- Maintain detailed documentation of all services provided, ensuring compliance with agency policies and state regulations.
- Ensure clients’ medication regimens are documented and that clients comply with their medication and treatment plans.
- Collaboration:
- Coordinate with healthcare providers, social service agencies, and other community resources to support client needs.
- Maintain regular communication with the Clinical Supervisor regarding client progress and any concerns.
- Act as a liaison between clients, families, and other service providers to facilitate comprehensive care.
- Quality Improvement:
- Participate in regular supervision, training, and professional development opportunities.
- Attend interdepartmental meetings and contribute to discussions on quality assurance and risk management.
- Ensure that all services provided adhere to high ethical and professional standards.
- Additional Responsibilities:
- Provide weekly availability and scheduling updates to the Administrative Team.
- Function as a mandated reporter for any suspected abuse or neglect.
- Participate in individual and group supervision sessions.
- Assist in program development and other duties as assigned by the Clinical Supervisor.
Minimum Knowledge, Skills, and Abilities:
- Strong knowledge of mental health principles, techniques, and community-based services.
- Understanding of case management practices, including resource coordination and service linkage.
- Ability to effectively engage and work with individuals from diverse backgrounds.
- Proficiency in clinical documentation and compliance with state regulations.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong communication, organizational, and problem-solving skills.
Education and Experience Requirements:
- Must meet the qualifications for a Qualified Mental Health Professional-Community Services (QMHP-CS) as defined by the Texas Health and Human Services Commission (HHSC). This includes:
- Education: A bachelor’s degree or higher in psychology, social work, counseling, or a related human services field.
- Experience: Relevant experience in providing mental health services to adults, children, or families in a community setting.
- Registration: Must be registered as a QMHP-CS with the Texas HHSC.
Additional Requirements:
- Must pass a criminal background check and have a valid driver’s license.
- Must complete a minimum of 20 hours of training specific to mental health services annually.
- Must have or be willing to obtain CPR & First Aid certification.
