Position Overview

Under the direction of the Clinical Director, the Case Manager is an individual, non-clinical service used to assist individuals in gaining access to needed medical, social, educational, and other services. The purpose of Case Management is to foster a person’s rehabilitation from a diagnosed disorder by organizing needed service and supports into an integrated system of care until the person served can assume this responsibility.

Full-time or part-time positions.

 

Essential Duties and Responsibilities

 

  • Facilitates community resource availability based on client needs to assure maximum social integration.
  • Assess client or family needs and eligibility for services.
  • Work with clients, families, and appropriate community agencies to coordinate and facilitate services for clients.
  • Evaluates, facilitates, and monitors services and client progress in relationship to established goals and objectives; documents promptly and timely in the EHR.
  • Monitors treatment plan as directed; provide follow-up and staffs areas of opportunity with Clinical Director and Primary Therapist.
  • Co-facilitate group sessions upon Clinical Director approval with proper training or education.
  • Strong understanding of and ability to engage in Case Management activities; Linkage, Monitoring & Follow-up, Referral, Advocacy, and Crisis Intervention.
  • Ability to plan and organize work effectively through flexible scheduling to meet the needs of clients and HCBH programs.
  • Consults and collaborates with Clinical Director to resolve conflicts related to scheduling services.


Candidate Qualifications and Experience

 

  • Bachelor’s Degree in Human Services field of study.
  • Prefer minimum 3 years’ experience in medical and Behavioral Health.
  • High degree of organizational skills and attention to detail.
  • Successful completion of HCBH pre-employment screening and background check.
  • Has the ability to communicate effectively orally and in writing.
  • Has a valid driver’s license and eligible for HCBH insurance coverage(s).
  • Proficient in Microsoft Office: Word, PowerPoint, Excel, Outlook.
  • Ability to conceptualize, develop and implement programs to their completion in a fast-paced environment.

While performing the duties of this job, the employee is required to walk, stand, sit, and use the hands. Occasionally stoop, crouch, or kneel. The employee must occasionally exert or lift up to 25 pounds.
