What are the responsibilities and job description for the Registered Nurse, Per Diem position at Highland Valley Elder Services, Inc.?
Highland Valley Elder Services is looking for a per-diem Nurse to conduct in-home clinical assessment with seniors to determine eligibility for programs. Shifts can be half days, full days, part time or full time.
Responsibilities:
· Conduct in-home Clinical Assessments with seniors.
· Determine Eligibility for a variety of Programs.
· Complete required documentation in State database to establish eligibility for programs.
· Provide ongoing interdisciplinary consultation with ASAP Case Management staff and area health providers regarding clinical eligibility criteria for ASAP and MassHealth programs and services.
· Attend applicable staff and/or agency meetings.
· Perform other tasks as assigned by supervisor to meet agency needs.
Job Types: Full-time, Part-time, Per diem, Temporary
Pay: $33.50 per hour
Expected hours: 4 – 40 per week
