What are the responsibilities and job description for the Social Services Designee position at Hillebrand Nursing & Rehabilitation Center?
Hillebrand Nursing and Rehabilitation is hiring a part time Social Services Designee.
Our Social Services Designee will work with residents in the facility by identifying their psycho-social, mental, and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.
Qualifications:
- Prior experience with the elderly in a long-term care facility
- Excellent communication, assessment and social work methods and techniques. Should be able to work effectively with a variety of disciplines in an individual and team setting.
Responsibilities:
- Assist with necessary documentation for the resident’s admission to the facility. To include, but not limited to the Social Service Evaluation, Initial Progress note/documentation, care planning and MDS assessments.
- Coordinates discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements.
- Comply with state and federal regulations.
- Attends administrative staff meetings, morning report, Performance Improvement meetings, resident care-related meeting.
- Maintains departmental records/documentation as required.
