We are currently hiring for a Director of Nursing (DON) for our Hilltop Healthcare location based out of Seminole. It is a senior leadership position responsible for overseeing the clinical operations and nursing staff of a home health agency. This role ensures that high-quality, compliant, and patient-centered care is delivered in accordance with state, federal, and accrediting body regulations. The DON collaborates with interdisciplinary teams to develop care plans, monitor outcomes, ensure staff competency, and drive continuous improvement in service delivery.
Responsibilities
- Recommends employment decisions for staff to the Vice President of Operations for Home Health, including decisions to hire, fire, advance, promote or change status.
- Responsible for overall quality of patient care and clinical performance of nurses and aides.
- Organizes clinical operations for the care center.
- Reviews requests for services and determines patient eligibility/suitability for home care services. Works with intake in coordinating patient referrals and admissions.
- Oversees scheduling.
- Oversees payor verification and precertification requirements.
- Reviews documentation of other staff members and ensures missing, incomplete, and/or untimely documentation issues are resolved.
- Assists clinicians in establishing immediate and long-term patient goals, setting priorities, and developing Plan of Care (POC).
- Regularly evaluates the needs of the patient and makes necessary revisions to the plan of care in collaboration with clinicians.
- May provide direct patient care on an infrequent basis in times of emergency.
- Participates in the on-call process.
- Assists with training of staff to ensure quality care, compliance, and fiscal responsibility.
- Participates in the investigation and resolution of patient/family/physician concerns.
- Serve as a crucial liaison between administration, nursing management, and staff.
Position Summary:
Educate, analyze, plan, coordinate and oversee service delivery. Evaluate member outcomes resulting from the care received.
Essential Functions:
- Identify member needs, goals and abilities

- Prepare and submit a comprehensive, written service plan for each member
- Implement service plans

- Orient and educate members
- Modify services to meet needs of members.
- Ensure confidentiality according to all relevant laws and regulations.
- Maintain adequate records
- Justifies expenditures and monitors and contains cost of plan.
- Assist in implantation of in-service education for field employees
SKILLS:
Excellent verbal, interpersonal and written communication skills; Excellent problem-solving skills; Ability to use a computer and relevant data bases, especially for required reporting; Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs
Previous leadership role, such as charge nurse or lead capacity, where you were responsible for interim reporting, issue resolution, setting direction, and having accountability for patient and staff results.

Qualifications
- Associate or bachelor's degree in nursing.
- Current Oklahoma RN license
- One year of clinical RN experience in home health.
- One year of RN management experience.
- Current CPR certification.
