RN Case Manager - Hospice

Job Summary :

We are looking for a compassionate and dedicated Hospice RN Case Manager to service the North Dallas area (Plano, Lewisville, Richardson, etc.). In this role, you will provide comprehensive, patient-centered care for individuals with life-limiting illnesses. The RN Case Manager will coordinate all aspects of hospice care to ensure comfort, dignity, and quality of life for patients and their families, while adhering to hospice philosophy and organizational policies.

Key Responsibilities :

Patient Care Management :

Develop, implement, and regularly update individualized hospice care plans for patients based on their needs, goals, and preferences.

• Provide skilled nursing care, including pain and symptom management, medication administration, and wound care, as appropriate.
• Perform physical and psychosocial assessments to identify patient and family needs.

Care Coordination :

Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and home health aides, to deliver holistic hospice care.

Patient and Family Support :

Educate patients and families about the hospice philosophy, disease progression, symptom management, and what to expect during end-of-life care.

Documentation and Compliance :

Maintain accurate, timely, and complete documentation of patient care and communication in accordance with regulatory and organizational standards.

On-Call Support :

Participate in on-call rotations to provide after-hours care and support as needed.
