Service Areas: We service 25 communities in southern NH from Hooksett to Nashua. We are the preferred provider for 3 hospitals.
Responsibilities:
The position requires in person meetings with patients and families in a hospital as needed. Additionally, they will participate in community collaborations with hospital partners.
- Electronic medical record experience preferably with EPIC.

Position can allow work from home, a hybrid schedule.
Candidate must have strong customer service skills.
Knowledge of hospice and home care, experience preferred.
Excellent communication skills and the desire to work as part of a team.
This is a part-time position, Weekends only.
- Qualifications
- Graduate of an accredited school of nursing as an LPN or RN.
- Nursing Diploma or Associate degree in nursing.
- Registered as RN or LPN with New Hampshire’s Board of Nursing.
- A minimum of two years’ recent acute care experience.
- Ability to exercise initiative and independent judgment.
Preferred Qualifications
- Bachelor’s degree in nursing.
- Community Health/Education experience.
- EMR software proficiency.
Required
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
- Bureau of Elderly and Adult (BEAS) clearance.
