Job Title: Care Manager
Job Overview:
In this role you will be responsible for overseeing the provision of high-quality home care services, ensuring that clients receive personalized care tailored to their needs. This role involves coordinating care plans, managing care staff, and maintaining effective communication with clients and their families. This is a full-time position that offers opportunities for professional growth and development.
Responsibilities:
- Develop and implement individualized care plans for clients
- Assess client needs and make necessary adjustments to care plans
- Coordinate with healthcare professionals, administrative staff and caregivers
- Monitor and evaluate the quality of care provided
- Supervise, train, and support home care staff
- Regularly communicate with client and/or designated client representative
- Perform regular follow-up on service delivery and ensure client satisfaction
- Ensure compliance with company policies and regulatory requirements
- Perform scheduled and non-scheduled in-home client support visits and assessments
- Investigate issues related to client care and client or employee well-being
- Handle client inquiries, concerns, and emergencies promptly and effectively
- Ensure records are current and properly maintained in accordance with agency policies and procedures
- Provide needed training and education to staff in the client’s home, via telephone and/or agency office
- Assist with the development and revision of client care policies and procedures
Qualifications:
- Bachelor’s degree in Nursing, Social Work, or a related field (preferred)
- Minimum of 2 years of experience in home care, home health or related field
- Knowledge of home care services and regulations.
- Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families
- Strong organizational and time-management skills.
- Excellent communication and interpersonal abilities.
- Compassionate and patient-centered approach.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office and care management software.
- Passionate about helping others live with dignity
- Must present a positive and professional business image
- Must pass a criminal background check
