JOB TITLE: Case Manager

SUPERVISOR’S TITLE: Program Manager

General Summary: The Case Manager position is responsible for creating a welcoming environment and be able to maintain a professional, kind, calm attitude for all of those engaging with Hope Harbor. As well as providing intensive case management to Hope Harbor clientswhich includes assessments, budgeting, referrals, education, and providing information about availablecommunity resources. This work is required to be documented through Clarity, a Homeless ManagementInformation Software (HMIS).

Qualifications:

  • Pass a complete series of background checks.
  • Possess an Associate’s Degree in Human Services or a related field, preferably with a Bachelor’s Degree, or equivalent experience.
  • Current CPR and First Aid Certification, or the ability to obtain these within thirty (30) days of being hired.
  • Be capable of accurately following written and verbal instructions.
  • Possess the ability to communicate effectively; orally, typed, and written.
  • Ability to be reliable and punctual.
  • Ability to work with persons in need in a compassionate and dignified manner.
  • Ability to maintain resident, staff, and volunteer confidentiality.
  • Must be able to work independently with a minimum amount of supervision.
  • Knowledge of computers and software, and the ability to accurately enter data within specific time deadlines.
  • Ability to carry on-call cell phone on a rotating basis

Provide Individualized Case Management

Provide intensive case management to residents that include identifying and setting goals &

monitoring client progress in achieving case plan goals. Develops individual case plans with

each family based on identified needs, information from assessments, and provides support

in reaching goals. Will document progress towards case goals and progress in our education

program. For families in shelter, assures families are following rules and regulations of

Hope Harbor programming. For those housed out of shelter, ensure they follow program

guidelines. Assists with employment readiness and retention, creating a debt repayment

plan, establishing individual family budgets. Candidate must be knowledgeable about

housing and education information in the community. Case managers may be asked to

facilitate education classes as part of Hope Harbor’s education programming. Case manager

will be responsible for linking residents to available community services in the area

including; but not limited to, housing, medical, mental health, child care, and financial

assistance.

Link Clients to Community Resources

Case manager will be responsible for linking clients to available community services in the

area including; but not limited to, housing, medical, mental health, child care, and financial

assistance.

Complete Accurate and Timely Documentation

Document case management in digital and physical files. Track statistics on client entrance,

exit, and program information. Follows expectations, timelines, and requirements of

Homeless Management Information System data entry.

Cross-Training

Candidate will cross-train with different departments to ensure an understanding of Hope

Harbor’s programming. This includes job shadowing, staff meeting attendance, and a focus

on communication

Professional Skills: Candidate will report for work on time or provide advance notice of

need of absence, efficiently produce quality work and within deadlines with minimal

supervision, manage workload effectively, have excellent communication skills – both

written and oral, engage in active listening with clients and co-workers, make thoughtful,

well-reasoned decisions and exercise good judgement, take initiative on projects and tasks,

identify problems and find solutions, contribute to an atmosphere of respect and make

valuable contributions to help the team meet our mission.

Job Type: Full-time

Pay: From $18.00 per hour

Expected hours: 40 per week
