Horizon Health and Rehabilitation Center is 138-bed skilled nursing facility, licensed by the State of Nevada, providing both long-term and short-term care. Our team at Horizon Health and Rehabilitation Center strives to provide patients with an optimal level of care based on their physician's orders. The goal is always to help our patients improve, and move to the next appropriate level of care - whether that’s skilled nursing care, home health services or a return to home without further services.
- Responsible for planning, developing, organizing, implementing, evaluating and directing of the social service programs of the facility
- Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents
- Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required
- Must have a current Social Work license, in good standing with the state (as required)
- Must have, as a minimum, one (1) year experience in a human services field in a hospital, skilled nursing care facility or other related medical facility
- Must be able to organize and prioritize many tasks effectively
- Ideal candidate must be a skilled communicator, director and motivator
