Job Summary:
We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our team. This role is responsible for providing high-quality palliative care to patients, collaborating with physicians and healthcare teams, and ensuring compliance with regulatory standards. The ideal candidate will have strong clinical skills, empathy, and a commitment to patient-centered care.

JOB FUNCTIONS
- Conduct patient visits as delegated by the Medical Director or Palliative Care Coordinator.
- Ensure adherence to The Perfect Visit Standards, including quality care, accurate documentation, and service excellence.
- Maintain accurate and timely patient records, reports, and documentation in compliance with agency policies.
- Monitor and report significant changes in a patient’s condition to the physician and appropriate healthcare personnel.
- Ensure compliance with all regulatory and accreditation standards.
- Demonstrate empathy and sensitivity to the needs of patients and their families.
- Perform comprehensive clinical care, including bedside evaluations, patient history assessments, physical exams, diagnostic interpretations, and case management participation.
- Educate clinical staff, patients, and families on disease management, prevention strategies, and care plans.
- Maintain strict confidentiality regarding patients, families, staff, and organizational matters.
- Assist with coordinating patient admissions and discharges as needed.
- Collaborate with physicians and healthcare providers to optimize patient care and provide written reports as requested.
Qualifications
Education:
- Graduate of an accredited school of nursing.
- Graduate of an accredited Nurse Practitioner Program.
Certification:
- Current Advanced Practice Registered Nurse (APRN) Louisiana license.
- Current Nurse Practitioner certification.
- Current CPR certification.
Special Skills:
- Ability to relate to people of diverse cultures and ethnic background.
- Must demonstrate maturity, initiative, tact and good judgment in dealing with staff, patients, and families.
- Knowledge of medications and sterile techniques.
- Knowledge of and appropriate use of PPE.
- Ability to work independently as well as on an interdisciplinary team.
- Supportive and sensitive to needs of patients and families, as well as other staff and volunteers.
- Humanistic attitude toward patient/family care.
Other Requirements:
- Good physical assessment skills and computer skills.
- Good communication skills and techniques with facilities, patients, families, physicians, and office staff.
