Position Overview

This Palliative Care Case Manager RN is part of a multidisciplinary team providing specialized medical care for people with chronic and/or complex conditions. While focused on community-based palliative care, this role will be cross-trained in hospice care to ensure a seamless transition for patients who may need end-of-life services.

 

Essential Duties and Responsibilities

  1. Provides nursing services to patients enrolled in Hospice of Humboldts Community Based Palliative Care Program, including patient assessments, interventions and evaluations. Nursing services will be provided in a variety of locations such as homes, long-term care facilities or via video-conference technology.
  2. Manages a caseload of patients, per agreed-upon standards
  3. Determines the overall plan of care in discussion with patient, caregiver, physician, and other members of the interdisciplinary team
  4. Documents assessment, interventions, and outcomes utilizing electronic medical record at point of care
  5. Coordinates and communicates with the primary care physician
  6. Triages calls from enrolled patients and families, assesses the needs of the patient and family, and coordinates needed care over the phone.
  7. May provide hospice nursing care including, but not limited to, the following:
    • Provides direct hospice nursing care to patients in their place of residence.
    • Functions as a hospice triage nurse to coordinates needed care over the phone
    • Develops, evaluates and adjusts the patients Plan of Care (POC) as indicated.
    • Collaborates with physicians and the interdisciplinary care team regarding patients changing condition.

Qualifications


Minimum Qualifications

  1. Graduate of an accredited nursing program.
  2. Current California Registered Nurse license.
  3. Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.
  4. Valid drivers license, proof of acceptable automobile insurance coverage, and reliable transportation.
  5. Reliable mobile phone with plan that provides good local coverage.
  6. Basic computer skills.

 

Preferred Qualifications

  1. Experience in hospice or home health.
  2. Experience working effectively with an interdisciplinary team.
  3. CHPN certification.

Salary : $41 - $50
