Position Title: Full-time Admissions Clerk.
The Admissions Clerk responds to referral calls from family and community resources to facilitate the referral and admission process, records complete and accurate information in the computer. In addition to these duties, distributes referral information to appropriate staff, refers all medical questions to the correct roles and possesses excellent time management and attention to detail skills. Very fast-paced environment that requires excellent multi-tasking skills.

We also know from caring for over 15,000 people that most people wish they had called sooner. Hospice of Huntington helps people with a life-limiting illness control the symptoms that keep them from living everyday life.

Duties
- Responds to referral calls from family and community resources.
- Records complete and accurate information in the computer.
- Contacts physician’s office to verify referral, and requests pertinent medical information to support the diagnosis.
- Tracks medical information that has been requested.
- Requests patient information that has not been received and notifies appropriate roles of any problems.
- Communicates with patients/family members to discuss and obtain necessary documentation.
- Communicates as instructed with referral sources and care partners to provide continuity of communication related to the referral.
- Assists in data collection.
- Contact private insurance company to certify insurance coverage and case management to verify hospice coverage; completes corresponding paperwork.
- Copies and distributes referral information to appropriate staff.
- Refers all medical questions to appropriate roles.
- Meets with families to educate on hospice services as needed.
- Manages the status of all referrals and the follow up needed.
- Other job duties as assigned.
- Minimum of 1-year patient care experience; hospice or home health preferred.
- Minimum of one year health care or customer service experience with demonstrated ability to establish good working relationships in person and by phone.
- Valid driver’s license and automobile insurance, and reliable transportation.
- Experience working with computer applications including Windows systems and the Internet.
- Knowledge of Medicare, Medicaid, and Private Insurance required.
- Ability to work in a quick paced, time sensitive environment.
