What are the responsibilities and job description for the Part Time RN Case Manager position at Hospice of Jefferson County?
Hospice of Jefferson County is seeking an immediate part time RN Case Manager interested in making a difference for those at the end of life. You will be part of an interdisciplinary team focused on palliative care and symptom management in the home or nursing home setting. A valid NY State RN License and drivers license are required. Competitive pay based on work experience and mileage reimbursement.
Job Duties include: The RN Case Manager will manage each patient’s care and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPNs) who will be directed / supervised by the RN. Overall responsibilities include planning, providing and coordinating quality end of life care.
Required Skills: Able to work independently, and complete documentation accurately and in a timely fashion. Valid driver’s license. Must be willing to travel throughout Jefferson County if required.
Job Type: Part-time
Pay: $37.00 - $39.00 per hour
