What are the responsibilities and job description for the Medical Records Clerk II position at Hospice of Southern Illinois, Inc.?

Essential Functions

  • Understanding the Hospice of Southern Illinois telephone system to include - answering calls, determining the purpose of calls, forwarding calls to appropriate staff members, taking/delivering messages and answering questions from the caller.
  • Competent to welcome on-site visitors, determine nature of business, and announce visitors to appropriate personnel.
  • Ability to enter the clinical on-call schedule and update as necessary.
  • Effectively utilizes Microsoft Office products to ensure accuracy and timely completion of tasks.
  • Working knowledge of process to file records in patient charts (timely and accurately) which includes preparing reconciliations and proper closure/storage of charts.
  • Ability to sort mail per organizational guidelines.
  • Abstracts diagnoses from Suncoast Solutions electronic record and enters in patient chart. Also assigned accurate ICD-10 code.
  • Ability to make revisions and updates to the organizational policies and standards of performance.
  • Ability to prepare deposit listings for use in bank deposit preparation.
  • Ability to perform professional license look up and notify appropriate departments of findings.
  • Ability to monitor and fax Medicaid 1592 and 1594 forms.

  • Position Summary: The Medical Records Clerk II shall be responsible for answering the phone and routing calls appropriately. The Medical Records Clerk II will also file all patient forms into the medical records, as well as reconciling and scanning of all patient charts.
  • Full-Time Non-Exempt Position
  • 40 Hours Weekly, Monday-Friday 8:30 A.M. - 5:00 P.M.
  • Would work out of the Belleville Administration Building located at 305 South Illinois Street, Belleville, IL 62220.

A.) E ducation

  • Graduate from an accredited high school.
  • Some college education preferred.

B. E x perience

  • Two years’ experience office environment preferred.
  • Knowledge of Windows Operating Systems and Microsoft Office.
  • Knowledge of medical record keeping procedures preferred.
  • Two years’ experience with telephone system.

C. *Other Qualifications

  • Appropriate and refined manners with telephone.
  • Accurate and neat with paperwork.
  • Ability to stand or sit for long periods of time.
  • Good organizational and verbal communication skills.
  • Knowledge of office equipment and computer.
  • Ability to operate a scanner and navigate a keyboard.
  • Excellent multi-tasking skills.
  • Knowledge of HIPAA and the importance of maintaining the confidentiality of patient information.
  • Ability to do alphabetical and numerical filing of medical records and journal entries.
  • Work well with other members of the team.

Physical Requirements: Performs repetitive tasks. Does sedentary work: Prolonged periods of sitting or standing. Requires clarity of vision, >20 inches and < 20 feet. Manual dexterity (eye/hand coordination). Hears alarm and telephone. Requires physical flexibility of bending at waist and knee frequently. Required lifting up to 25 pounds occasionally. The Medical Records Clerk II manages stress appropriately, manages fear/anger/hostility/violence of others appropriately and handles multiple priorities. Bending at knees and waist.
Working Conditions: T he Medical records Clerk II spends approximately 99% of his/her time in an air- conditioned environment with varying exposure to excessive humidity and noise. The remaining 1% of his/her time is spent in an automobile that may or may not have heat/air conditioning.
