Nurse Navigator - FT

Healthcare professional who helps patients navigate the healthcare system from diagnosis, through treatment and sometimes end-of-life care. This person will act as a bridge between patients and clinical care staff providing personalized care to patients, families and caregivers. They help with connecting patients to medical and social resources. Follows up with patients regarding regularly scheduled appointments and referral appointments. Provides follow up to health care providers, agencies and other resources about patient progress towards meeting needs and/or goals.
