JOB TITLE: Community Support Assistant (PT)

SUMMARY:
Provides support to young people and families to assist them in improving the skills necessary to become independent and self-sufficient.
Supports the activities and goals outlined in the housing stabilization and individualized services plans through independent living instruction and mental health interventions.

- Ability to work autonomously, manage shift time to meet client needs, and identify/communicate issues to Program Manager.

MAJOR RESPONSIBILITIES / ACTIVITIES
1. Responsible for understanding housing stabilization and treatment plans prior to meeting with clients.
2. Utilizes the plans and independent living assessment information to identify areas of mental health intervention and support that will increase independence and functionality of youth and families.
3. Supports CSPs in teaching independent living skills such as budgeting, banking and money management, grocery planning and shopping for nutritional meal preparation, household maintenance skills, and options for accessing community healthcare, transportation and housing.
4. Provides mental health interventions that focus on the individual’s ability to succeed in the community.
5. Works closely with young people to develop their skills in the areas of problem solving and anger management.
6. Supports Crime Victim Specialist in providing interventions and engagements using a victim services framework.
7. Maintains excellent up-to-date skills in suicide intervention, crisis de-escalation and trauma informed care.
8. Assists youth with accessing available resources in the community.
9. Seeks support in all situations where there is a question or hesitation regarding one’s actions or role with a young person.
10. Plans and conducts group sessions with clients to address independent living skill development, mental health issues and to promote social skill development and community connections.
11. Completes expectations of a shift by completing any checklists, safety checks, follow-ups and engagements with youth.
12. Maintains required caseload and/or meets established productivity standards.
13. Accurately completes documentation in a timely manner.
14. Completes other duties as assigned.
Requirements:1. HS Diploma or GED.
2. Strong verbal and written communication skills.
3. Computer literate with strong data entry skills.
4. Ability to relate to and have empathy for young people.
5. Holds and maintains a valid driver’s license with a good driving record and valid insurance.
