What are the responsibilities and job description for the Social Worker position at Hudson Park Rehabilitation and Nursing Center?
Social Worker
Schedule: Full time, Tuesday thru Saturday or Wednesday thru Sunday approximately 8:30 am - 5:00 pm. Late nights on occasion. This is a salaried, exempt position.
Duties/Responsibilities:
Ability to provide supportive counseling and education to residents and families as needed.
Actively participate in resident care conferences and complete documentation in accordance with facility guidelines.
Knowledge of long term care rules and regulations required.
Deals well with a diverse client population ranging from the geriatric population to younger rehab residents with varying physical/social issues.
Works well in a busy environment with multiple competing priorities.
Requirements/Qualifications:
Two years experience as a Social Worker dealing with the adult/geriatric population, preferably in a long term care setting.
Degree in Social Work or closely related field beneficial.
