What are the responsibilities and job description for the Forensic Case Manager position at Human Services Administration Organization?

Provides direct and indirect services for Persons involved in the Criminal Justice System who are diagnosed with a mental health disorder. Major responsibilities include independent assessment (to include ASAM), monitoring, advocacy, and follow-up for a specified caseload as consumers move throughout the mental health and criminal justice system. This person must also work in collaboration with the Service Coordination Units, and the rest of the Specialty Court team members - Probation Office, District Attorney’s Office, Public defender’s Office. This position will work under the direct supervision of the unit Supervisor.

Duties And Responsibilities

  • This person is to communicate with the Specialty Court Judge and Probation Officer, for all the defendants in the Specialty Court Probation/Parole system. This person will assist in keeping the Judge updated about defendant progress and the problems they are having by acting as the link between Justice Related Services Specialty Court Liaison and the defendant on probation. This person will help schedule positive and negative reinforcement hearings with Judge’s staff
  • The person works with the Specialty Court Judges, the unit Supervisor, PD and ADA and is to aid or assist in any matters assigned to the Court
  • Implements and provides services based on client need:
    • Be the “go-to” resource for the client served
    • Assure that there are effective “safety net’ resources for the client served
    • Clearly communicate to the client what they can expect from the system and what the system will expect of them
    • Assure there is periodic assessment and cross system planning to meet the client’s needs while utilizing their strengths
    • Prepare for, convene/facilitate service planning meetings and provide follow-up after meetings
    • Assure there is cross system coordination of services and that services are being provided
    • Assist the client in developing and using natural supports
    • Be a persistent advocate for those served and give feedback on systemic problems
    • Provide a consistent positive outlook that encourages recovery and full inclusion in the community
  • Meets productivity expectations and complies with all policies and procedures of HSAO Justice Related Services
  • Effectively and efficiently manages a caseload as evidenced by adhering to all standards, maintaining current records, turning all notes in daily, completing all court service plans and authorizations as per established time frames, and is present and prepared for supervision
  • Collaborate with the SCU, Treatment Team (i.e Torrance State Hospital Regional Forensic Psychiatric Center, Allegheny County Jail Mental Health Unit, U.S. Western District (Federal) Court and Probation, PA Department of Corrections, PA Board of Probation and Parole, Allegheny County Adult Probation, Court of Common Pleas) and other service providers in a multi-disciplinary approach
  • Assist the consumer in locating and obtaining needed services as needed
  • Participate in discharge planning from Torrance State Hospital Regional Forensic Psychiatric Center, Allegheny County Jail, PA Department of Corrections and/or U.S. Bureau of Prisons to assure appropriate referrals and appointments are obtained
  • Collaborate with the SCU to assist the consumer in obtaining appropriate housing, utilizing contingency funds when needed.
  • Monitoring of service delivery
  • Contact provider agencies at a minimum of once per month to determine status of progress with the service plan
  • Participate in ongoing training in the mental health, drug & alcohol, and criminal justice fields
  • Communicate with various provider organization/agencies the problems or consumer complaints and offer feedback to the consumer
  • Will attend Specialty Court Workgroup Meetings to discuss policies and procedures regarding Review Hearings
  • Provide technical assistance to OBH service providers as it relates to the application of the legal requirements of the criminal justice system and the behavioral health service system to defendants with behavioral health disabilities in case planning
  • Provide consultation and review of difficult or complicated cases involving defendants with behavioral health disabilities. Also provides recommendations for case planning and referrals for services
  • Perform other duties as assigned
SKILLS AND KNOWLEDGE

  • Ability to work in a team-oriented atmosphere
  • Ability to be attentive to detail
  • Demonstrated organizational skills and meet all deadlines with all paperwork.
  • Ability to be a self-starter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise
  • Knowledge of Microsoft Office products including Word, Excel, and PowerPoint
  • Ability to adapt to learn new software products
  • Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities
  • Good communication skills

ESSENTIAL REQUIREMENTS

  • Possess Act 33 and 34 Clearances
  • Upon hire employee must obtain Institutional Clearances through the National Crimes Informational Center (NCIC) conducted by The Internal Affairs Dept of the Allegheny County Jail
  • Obtain yearly Institutional Clearances through the National Crimes Informational Center (NCIC) conducted by The Internal Affairs Dept of the Allegheny County Jail
  • Have a valid Pennsylvania motor vehicle license, access to a personal vehicle, and have maximum auto insurance on an employee driven vehicle
  • Ability to work independently
  • Full time position requiring 38.75 hours per week

Education And Experience

Master’s Degree and one year work experience working directly with persons with behavioral health needs or persons involved in the criminal justice system, OR a Bachelor’s Degree and two years work experience working directly with persons with behavioral health needs or persons involved in the criminal justice system.
