Job Description:
As a Social Service Instructor at our skilled nursing facility, you will play a crucial role in providing support and guidance to residents and their families. Your primary responsibility will be to assess the social and emotional needs of residents and develop individualized care plans to address those needs.
Responsibilities:
1. Conduct comprehensive assessments of residents' social and emotional needs upon admission and regularly throughout their stay.
2. Develop and implement individualized care plans for our long-term custodial in Medicare patients in collaboration with the interdisciplinary team, including nursing staff, therapists, and physicians
3. Provide counseling and emotional support to residents and their families, addressing issues such as adjustment to the facility, coping with illness or disability, and end-of-life planning.
4. Advocate for residents' rights and ensure that their preferences and wishes are respected in all aspects of care.
5. Coordinate with community resources and agencies to provide additional support and services to residents as needed, such as financial assistance, housing placement, or legal aid.
6. Working in conjunction with psychiatrist to do monthly medication review and every patient has an advanced directive.
7. Maintain accurate and up-to-date documentation of assessments, care plans, and interventions in accordance with regulatory requirements and facility policies.
8. Participate in interdisciplinary team meetings, case conferences, and staff training sessions to enhance communication and collaboration across departments.
9. Stay current with developments in social work practice, aging services, and regulatory requirements, and incorporate best practices into service delivery.
10. Collaborate to ensure smooth discharge planning for residents returning home or transferring to other care settings.
Qualifications:
1. Bachelor's or Master's degree in social work or a related field.
2. State licensure or certification as a social worker (if applicable).
3. Previous experience working with older adults in a healthcare or long-term care setting preferred.
4. Strong assessment, counseling, and communication skills.
5. Ability to work effectively as part of a multidisciplinary team.
6. Compassion, empathy, and patience in working with residents and families facing challenging situations.
7. Knowledge of community resources and social service agencies.
8. Proficiency in documentation and record-keeping.
9. Understanding of relevant regulations and standards governing long-term care facilities.
10. Commitment to maintaining professional boundaries and confidentiality.
