Nurse Practitioner (NP) – Nephrology PracticeOverviewThis is an excellent opportunity for a Nurse Practitioner to join a collaborative nephrology practice focused on delivering high-quality, patient-centered kidney care. The NP will work closely with nephrologists and interdisciplinary teams to manage patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD), emphasizing education, care coordination, and comprehensive disease management.Key ResponsibilitiesClinical Care & Patient ManagementProvide ESKD patient rounding, including limited and comprehensive visits, under the guidance of nephrologistsDeliver holistic ESKD care management, including:Patient activation and engagementDepression screening and supportHigh-risk hospitalization oversightTransitions of careManagement of co-morbid medical conditionsConduct annual comprehensive ESKD disease management visitsProvide CKD clinical care and kidney disease educationCare Coordination & CollaborationCollaborate with nephrologists, dialysis center interdisciplinary teams, RN care managers, hospital discharge teams, primary care providers, and specialists as clinically indicatedParticipate in care planning and communication across the continuum of care to support optimal patient outcomesClinical FunctionsPerform physical examinations and assess patient health statusOrder, interpret, and evaluate diagnostic tests and laboratory studiesDevelop, implement, and adjust treatment plans as neededPrescribe medications in accordance with scope of practice and state regulationsDocument patient encounters accurately within the electronic medical recordEducate and counsel patients and families on diagnoses, treatment plans, preventative care, and healthy lifestyle behaviorsProfessional PracticePromote a high standard of patient service and professionalismBuild effective working relationships across clinical and operational teamsParticipate in team-based care and adhere to organizational policies and proceduresMaintain regular, reliable attendanceQualificationsRequiredCurrent Nurse Practitioner (Advanced Practice) license in the state of practiceAppropriate specialty certificationMinimum of 2 years of clinical experienceStrong understanding of physician practice operations, protocols, and workflowsExcellent written and verbal communication skillsPreferredPrior nephrology and/or dialysis experienceBenefitsMedical and pharmacy coverageDental and vision insuranceLife insuranceVirtual care servicesVoluntary benefits401(k) with employer matchPaid Time Off (PTO)
