Job Summary
The Ideal Center for Social Impact is seeking a dedicated and experienced Case Manager to support our grant-funded social work programs. This is an immediate hiring need for a critical role that works directly with students and program participants to ensure they remain motivated, supported, and positioned for long-term success.
The ideal candidate will bring strong case management experience, a passion for social services, and the ability to manage participant progress under grant requirements.
Key Responsibilities
- Create, implement, and maintain Individual Employment Plans (IEPs) for participants
- Work closely with students/participants to ensure engagement, motivation, and goal attainment
- Identify, document, and track participant barriers, progress, and outcomes
- Vet and assess students for eligibility into Ideal programs
- Refer participants to third-party agencies for additional supportive services as needed
- Complete timely and accurate case notes, documentation, and data entry in accordance with grant requirements
- Ensure compliance with program guidelines, reporting standards, and deadlines
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (required or preferred)
- 2 years of case management or social services experience preferred
- Experience working with grant-funded programs strongly preferred
- Strong documentation, organisational, and time-management skills
- Ability to build rapport and work effectively with diverse populations
- Proficiency in data entry systems, case management software, and Microsoft Office

Pay: $22.00 - $25.00 per hour
