Impact Behavioral Health Partners, a community mental health agency located in Evanston, seeks a Clinical Case Manager to serve as a member of a multidisciplinary team that helps people living with mental illness.


As a Clinical Case Manager, you will provide a range of strength-based, person-centered services to participants living in Impact’s housing program. These services will promote the participants’ recovery, independence, and community integration. You will work with many individuals who may be more challenging to engage. This role will require increased outreach efforts and close collaboration with our housing and clinical teams to ensure service delivery.


We are looking for a self-starter who has excellent communication abilities, outstanding organizational skills, and can work well independently. The Clinical Case Manager reports to the Clinical Program Supervisor.


Salary: Starts at $45,000 per year, bonus pay is possible.

Paid Time Off

  • HMO or PPO health insurance through Blue Cross Blue Shield is provided at no monthly premium cost to the employee, for plans that do not include a spouse or children.


Dental and Vision Insurance

  • Dental and/or vision insurance is available at affordable monthly costs.


401(k)

  • Impact will match 401(k) contributions, up to 2% of an employee's salary.


Employee Assistance Program (EAP)

  • The EAP provides each employee with up to 5 free counseling sessions per problem, per year, at no cost to the employee.


Mileage Reimbursement

  • Employees are reimbursed 67 cents per mile driven for work-related purposes, not including commuting to and from our administrative office in Evanston.


Location: This is an in-person role. It will primarily be on-site at Impact’s housing properties in Evanston, Skokie, and other scattered site locations in northern suburban Cook County. Some meetings and administrative work will be done through video conferencing. The ability to work remotely from home cannot be guaranteed.


Schedule: Full-time position, 37.5 hours per week. Flexible scheduling may be available to accommodate employee needs outside of work but cannot be guaranteed. Staff may end their workday at 1:00 pm on Fridays if they have successfully completed their weekly job requirements.


Primary Job Responsibilities:

● Provide a range of direct services that include but are not limited to case management, resource connection, team-based case coordination, crisis intervention, and discharge planning.

● Assist participants in applying for and maintaining public benefits such as Medicaid, SNAP, Social Security, etc.

● Independently manage a caseload of 25 to 35 individuals that are regularly engaged. Manage check-ins and outreach for other individuals that are disengaged.

● Complete documentation of all relevant participant activities, such as progress notes, initial and on-going assessments (IM CANS, LOCUS), crisis assessment/safety plans, critical incident reports, etc. in a timely manner.

● Meet all established caseload and billing requirements.

● Conduct outreach and engagement activities for participants such as presentations, door knocking, cold calling, etc.

● Build and maintain relationships with partner organizations and community agencies through outreach and engagement activities.

● Integrate effectively with Impact’s housing and clinical teams to collaborate around participant care. Help teams to develop case management strategies for each participant.

● Fully promote and participate in the integration of Housing First, harm reduction, and trauma-informed care approaches across all services.


Qualifications:

● Bachelor’s degree in social work, psychology, sociology, or related human service field required.

● One to two years relevant social services experience preferred, internships or employment.

● Experience with persons with a chronic mental illness, preferably in a housing or multidisciplinary setting.

● Demonstrated experience in and commitment to providing services from a harm reduction approach.

● Must be an effective communicator, both verbal and written. Spanish speaking ability is preferred.

● Must be an effective problem solver, approach challenges in a positive manner.

● Must be able to demonstrate outstanding organizational skills. This person needs to be able to maintain confidentiality while being committed to accuracy, thoroughness, and attention to detail.

● Demonstrated experience in balancing team and individual responsibilities. Must be able to contribute to a positive team spirit, boost morale, contribute to accomplishing team goals and objectives, encourage growth amongst peers, and give and receive collegial feedback.

● Must be able to utilize Microsoft Office Suite, Google Workspace Suite, Electronic Health Records, and other pertinent software.

● Ability to design workflows and procedures, as well as commitment to improving quality of processes and services.

● Customer service experience is a plus.


Additional Job Functions:

● Occasionally be available to work flexible schedules. In some instances, this is required as an accommodation for participants with special needs. In other instances, flexible scheduling may be necessary to assure adequate clinical team coverage.

● The Clinical Case Manager is required to have a valid Illinois driver's license and access to a reliable car.

● Employees are required to meet agency standards in their personal vehicles for milage reimbursement. This includes but is not limited to employees personally obtaining and maintaining $100,000 /$300,000 bodily injury and liability auto insurance policy. Proof of insurance must be provided at least annually, and a department of motor vehicles check will be run on all employees’ driver’s license at least annually.

● Other duties as assigned.

● Ability to twist, turn, bend, stand, climb stairs, and lift as required to perform the duties associated with this job.
