JOB DESCRIPTION
JOB TITLE: SOCIAL WORK CARE MANAGER II
REPORTING UNIT: SUPERVISING CARE MANAGER
STARTING SALARY: NON-EXEMPT RANGE VI
($18.91-$24.14 PER HR DOE)
PAY UNIT: TWICE MONTHLY
HOURS: FULL TIME; SCHEDULED AS NEEDED
MINIMUM QUALIFICATIONS:
1. Bachelor’s degree in social work, nursing, psychology, counseling, rehabilitation, gerontology, sociology or related field required.
2. 3 years’ experience with the elderly and Master’s degree in the above field preferred.
3. Ability to meet the public effectively and knowledge of community programs and resources.
4. Excellent problem solving skills.
5. Ability to conduct in-depth assessments, conduct follow-up and monitor progress.
6. Reliable transportation available for work.
7. Knowledge of community resources.
8. Professional communication skills, oral, and written, and Bilingual (English/Spanish).
9. Valid California Driver’s license and minimum state auto insurance coverage.
10. Must be insurable with ICWTC auto insurance policy.
11. Treat all others with respect and dignity.
12. Must pass a background clearance through Department of Justice.
DUTIES AND RESPONSIBILITIES:
1. Must conduct in-depth psycho-social assessments/reassessments covering psychological, environmental, rehabilitative, and social concerns. Reassessments must be completed in a timely manner by the anniversary month of the original assessment.
2. Maintain a caseload of fifty (50) active program participants per month.
3. Ensure that all program case files are compliant and ready for review per program and agency standards.
4. Responsible for ensuring timely case recording, documentation and monthly contact to include but not limited to re/assessments, care plans, progress notes, terminations, etc.
5. Identify and address functional limitations which pose barriers to independent living and refer to consultants for evaluation and recommendations for improving client functional level and quality of life.
6. Must develop, implement, coordinate services and monitor care plans that address client needs identified during assessments/reassessments.
7. Must identify, develop and enhance client support systems.
8. Must input client data on monthly basis into client tracking system.
9. Responsible for compliance with CARF standards related to all Agency programs.
10. Represents and interprets the agency at community conferences and meetings.
11. Informs SCM of situations of crisis or circumstances where MSSP staff requires further guidance
12. Implements program objectives and performance standards as directed by supervisor/director.
13. Assist MSSP staff subject matter to include but not limited to enrollment, termination, delivery of item, care planning and case file procedures.
14. Acceptance of other duties as assigned.
