Independent Living Partners is seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a vital role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to facilitate positive outcomes. The ideal candidate will possess strong skills in waiver case management, care coordination, Targeted Mental Health Case Management, crisis management, motivational interviewing, and workforce development, with a commitment to enhancing the well-being of individuals served.
We are seeking a proven leader with extensive waiver case management experience who can effectively collaborate with a seasoned team in disability services
Key Responsibilities
- Accept and process referrals; complete client intakes and discharges within established timelines
- Assess areas in the client’s life where assistance is needed
- Conduct thorough assessments of clients' needs and strengths to develop tailored service plans
- Utilize motivational interviewing techniques to empower clients in their decision-making processes
- Inform clients or legal guardians of service options that best meet their needs
- Monitor client progress and adjust service plans as necessary to ensure ensure appropriate and effective service delivery
- Assist clients and their families in accessing services, resources, and informal supports: including medical, social, educational, and other services regardless of funding source
- Coordinate services between providers and clients
- Conduct annual renewals, semiannual visits, and monthly check-ins per waiver guidelines
- Coordinate services with interdisciplinary teams to ensure continuity of care
- Maintain accurate and up-to-date documentation in compliance with lead agency and regulatory requirements
- Maintain required billable hours; complete documentation promptly; and ensure client progress is tracked and communicated with the interdisciplinary team
- Provide crisis assessment and intervention to support client safety and well-being
- Provide oversight and leadership for the case management program
- Supervise and mentor new staff or interns as needed to enhance team performance and client care
- Conduct file audits to ensure regulatory compliance
- Complete internal investigations and handle maltreatment reporting
- Conduct quality assurance audits and assist with licensing surveys
- Perform all case manager duties and responsibilities as required by DHS and county
Strong knowledge of community resources, disability waivers, targeted mental health case management, Veteran Services and service coordination

Minimum Qualifications
- A bachelor's degree in Social Work, Special Education, Psychology, Nursing, Human Services, or another related field focused on the education or treatment of individuals with developmental disabilities, mental health or related conditions.
- 2 years of waiver (CADI, BI, CAC, DD) case management experience
Skills
- Experience in waiver case management, targeted mental health case management & care coordination is highly preferred
- 4 years of experience as a 245D Designated Coordinator
- Proficiency in crisis intervention strategies and behavioral health practices.
- Familiarity with disability services
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail for effective case documentation.
- Ability to work independently as well as collaboratively within a multidisciplinary team environment.
- Commitment to ongoing professional development and staying current with best practices in case management.
