JOB SUMMARY
Assist individuals with a disability to lead a self-directed life and contribute to the community, assist with activities of daily living and encourage attitudes and behaviors that enhance community inclusion. A DSP provides these supports for an individual at work and other community places. Act as a facilitator for the individual, in communicating their needs, self-expression, and goals.

ESSENTIAL DUTIES & RESPONSIBILITIES FOR A DSP
- Support and provide individuals with meaningful community activities that are of interest to them.
- Transport persons served using their own personal vehicle and ensure their safety out in the community.
- Assist in developing, implementing, and monitoring Individual Service Agreement (ISA).
- Maintain accurate records including daily logs, mileage, individual files, accident and incident reports.
- Maintain open and focused communication with team members.
- Follow established schedules for individuals supported in the program.
- Project a professional image at all times.
- Provide behavioral management and support.
EDUCATION & EXPERIENCE REQUIREMENTS FOR A DSP
- Minimum age is 18 years old
- Possess high school diploma or equivalent
- Experience working with individuals with disabilities preferred
SKILLS & ABILITIES FOR A DSP
- Possess a valid driver’s license without restrictions
- Have a reliable vehicle with the ISN required automobile insurance coverage of $100,000/$300,000
- The ability and willingness to travel to off-site locations
- Ability to pass a criminal background check, DMV drivers record check, BEAS (Bureau of Elderly and Adult Services) background check, and national background check
- Be computer literate and have English writing skills
- Flexibility in scheduling
WORKING CONDITIONS & PHYSICAL DEMANDS FOR A DSP
- Ability to stand/walk for up to 6 hours a day
- Ability to perform a range of motions/tasks, including but not limited to; reaching, bending, stretching, stooping and lifting.
- Ability to lift 50 pounds
